Thanks to all for this information.

The box says DHE 28 ST-2-BT Parker double barrel combination shotgun beaver tail forearm, regular stock
28 gauge
26" barrel ic-mod
28" barrel mod & full

There is a # 1 on the barrel lug

so I assume it was a 2 barrel set originally.
