Forearm comes off in hand when fired

Default Forearm comes off in hand when fired

I have a DH that when I shot doves with it (lots of shots) the forend detaches from the barrels. If one just barely pulls the lever, it comes off easily. But not firing it is on there and does not fall off.

Looking for the repair (hope it is easy). No, I don't want to run Scotch tape around the barrels and forend! Thanks.
