The word "need" is a relative term. The shooter's physical characteristics which are so pronounced as to become a limitation, mean he would 'need' cast in his stock. What is 'Cast Off' for the masses, is Cast On for the smarter left handed shooters.
I have a Webley & Scott 12 bore that must have belonged to Andre the Giant, or his British cousin, Sir Giant. It was bent with about 3/8" cast ON for him because of the width of his head (picture a Basketball), but since I'm a southpaw, it's cast off for me, and that gun fits and shoots for me beautifully. My lawyer buddy (not George) is a righty, and has a head full of lawyerly things so he, too, can shoot it, but a normal righty would find themselves leaning way out to get their eyes aligned with the top rib.
