You can have the full choke opened concentrically. I have an Ithaca Classic Double 28 ga. that shot low even though it had a high comb, and I wanted the chokes opened up anyway, so Briley said they could change the point of impact by opening the chokes off-center. It worked perfectly. If there's enough metal (there was not in the ICD), they can also install an off-center thin-wall choke tube.
