Research Letter Information

I am preparing to send in my research letter request and was looking for some assistance in completing it out correctly. How do I determine the grade? Action type? Fore end type? Butt type, on the rib it's stamped Vulcan Steel

133683 appears to be the serial number, although the "6" looks damaged a bit.

The grade is VH

All I really need is the S/N. The rest of the fill in information is not necessary but it helps to make sure what the books say versus what you have on the form match. People do put down the wrong S/N occasionally.
