At least with a Perazzi you can take the barrels off , remove the extractor and get the shell out.
The old Peter's Blue Magic were notorious for separating if reloaded too many times.

The one CG I had was a 12ga and to me it seemed the chambers were big. Though I didn't have this problem you describe but I remember ammo fell into the chambers.

My Redheaded Stepchild ( an O/U I built) is quite temperamental regarding what ammo is used. It was a box of parts, in the white that I had to fit each and every one and make wood for. Its a bit crude and no chokes.
