Do you know what model this would be?

It is a top action hammer gun. This would likely be considered an 0 grade at the time, but could be a grade 1. Both grades would have little engraving and have twist steel barrels. This gun would have been after the time of 0 grades having NO engraving at all.
A letter would be the only way of knowing the grade for sure. Your gun seems to lack checkering, which even an 0 grade of this period would usually have checkering on it. The checkering on your gun was likely worn or sanded off. Value is really dependant on overall condition. Yours is in fair condition by the looks of it and I would assign a rough market value of $500 on it. Now, if the gun has very nice bores, or is a light frame size, the value could be higher to some.

What gauge is it? If it is a 12, I agree with Brian. If a 16 or other gauge, value might be higher

It's made on a 1-frame and is probably a 12 gauge.
