As close as that flaw is to the surface, this should have been spoted as a flaw. When I'm selecting a blank & I spot a crack or flaw I use a long needle to probe the flaw. They can go very deep. Although, I have cut into a perfectly flawless blank & they ended up looking like swiss cheese. Sometimes it's just the luck of the draw.
